Non-urgent advice: Sick notes
For a first sick note you will usually need a telephone appointment. Please call the practice on 01642 745800 to arrange this.
For repeat sick notes you can submit a request through our online consultation service -
All requests will be passed to a GP, however this does not guarantee it will be completed and that the patient may need a review before any further sick notes can be issued. These sick notes will be backdated.
Non-urgent advice: Letters
Yarm Medical Practice can provide private letters. This is non NHS work and you will be charged for private letters. The fee must be paid before the letter can be started.

- Speak to reception in person
